Wound Prevention and Documentation

Summary of course: There are an estimated 2.5 million patients per year that get a stage 2 or greater pressure injury. Pressure injuries cost $9.1-$11.6 billion per year in the U.S., and the cost of individual patient care ranges from $20,900 to $151,700 per pressure injury. Additionally, more than 17,000 lawsuits are related to pressure injuries annually. It is the second most common claim after wrongful death and has more claims than falls or emotional distress. About 60,000 patients die as a direct result of a pressure injuries each year. Aggressive pressure injury prevention, along with accurate wound assessment and documentation, are a top priority for staff and facilities alike, to implement a foundation for any wound prevention and treatment program.

During this seminar, we will:

  • Discuss pressure injury prevention and person-centered care
  • Learn parameters for wound assessment
  • Describe wounds using the red/yellow/black documentation system
  • Define severity of wounds by defined staging systems and levels of tissue loss
  • Describe etiologies of various wounds

This seminar will not cover wound treatment techniques. This seminar is meant to give the attendee a basic knowledge of the standard of care practice for wound prevention, assessment and documentation ONLY.
