Course Description:

In the delicate realm of end-of-life care within SNFs/NFs, the importance of supportive and empathetic documentation cannot be overstated. This presentation is crafted to assist healthcare professionals with the essential skills and knowledge needed to provide comprehensive and compassionate documentation during the final stages of life.

Learning Objectives:

1. Participants will acquire the skills to create thorough and compassionate documentation including care preferences, goals, and interventions specific to end-of-life scenarios.

2. Participants will gain an understanding of the regulatory and ethical considerations associated with end-of-life care documentation.

3. Participants will understand how to document the holistic aspects of care, including psychological and spiritual dimensions, to ensure a dignified and compassionate end-of-life experience for residents and their families.

Course Content:

15 min - Explore the foundational principles and key components of documenting end-of-life care and the importance of capturing resident’s preferences and wishes.

15 min - Discuss the regulatory and ethical frameworks governing end-of-life care documentation, advance directives, and respecting patient autonomy.

15 min - Understand how to capture holistic aspects of care, including psychological and spiritual dimensions, to ensure a dignified and compassionate end-of-life experience.

15 min - Discuss best practices for documenting pain management and care adjustments in a manner that supports comfort and quality of life.
